As a part of fulfilling the requirements for my Master of Arts in Music & Worship, I am conducting research on the cause and effect relationship between worship and discipleship. To fill out the online questionnaire, please click on the link below.

While it is possible to complete the questionnaire on a mobile device, it is probably best to do so from a laptop or desktop computer. Please allow approximately 30 minutes to complete the entire questionnaire at once. Responses are not saved until the questionnaire is complete and you click “submit.” Unfortunately, there is no way to complete a portion, save the results and resume at a later time.
